As a Home Health Licensed Practical Nurse (LPN) with Adoration Home Health and Hospice, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. Key Responsibilities:- Provide direct care to patients in accordance with a comprehensive care plan in the home care setting
- Document patient services accurately and in a timely manner
- Assess and educate patients and their families/caregivers on health-related topics
- Participate in the coordination of home care services and quality assessment activities
- Ensure compliance with state and federal regulations
- Graduate of an accredited practical or vocational nursing program with current licensure to practice in the state of operation
- 1 year of nursing experience required, 3+ years preferred
- Experience in community health, hospice, or med/surg preferred
