POSITION SUMMARY
This part-time role is integral in delivering direct support to clients enrolled in our Outpatient Treatment (OT) and Intensive Outpatient Programs (IOP). The counselor will play a crucial role in guiding clients towards the development and maintenance of healthy lifestyles and effective recovery management skills.
The position requires a comprehensive understanding of our clinic's treatment philosophy, admission protocols, and criteria. A foundational knowledge of substance use disorders and co-occurring conditions is essential. The counselor will ensure that both clinical and administrative services are effectively delivered to patients and their families.
Key Responsibilities:
- Manage a designated caseload in compliance with relevant regulations.
- Ensure that new clients receive a complete psychosocial history and a thorough assessment within five days of their admission.
- Collaborate with the team to create a Preliminary Treatment Plan within three days of admission.
- Work with clients and the team to develop a Comprehensive Treatment Plan for all patients within 30 days of admission.
- Conduct Treatment Plan Updates every 30 days following the initial Comprehensive Treatment Plan.
- Prepare aftercare plans for patients completing the program or being discharged for other reasons.
- Provide follow-up care and maintain contact with each discharged patient.
- Deliver individual counseling sessions focused on addiction recovery as outlined in the Comprehensive Treatment Plan.
- Coordinate Health Home services in alignment with the Treatment Plan.
Qualifications:
Applicants must possess trainee status from the Board of Professional Counselors and Therapists or hold certification as a licensed addiction counselor (LGPC, LCPC, CAC, or higher).
