About the Role

CarePartners is seeking a skilled Physical Therapist to join our Home Health Franklin Team. As a member of our team, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes.

Key Responsibilities
  • Manage an assigned caseload of patients, providing comprehensive physical therapy and clinical case management to improve patient outcomes and manage resources effectively.
  • Conduct thorough assessments, develop and modify plans of care in collaboration with patients, caregivers, and physicians to achieve patient goals and clinical outcomes.
  • Provide treatment visits and discharge planning to facilitate patient engagement and achieve positive patient experiences.
  • Communicate effectively with the care team, including physical therapists, physical therapist assistants, and other healthcare professionals, to ensure seamless care coordination.
Requirements
  • Current North Carolina license to practice Physical Therapy
  • Driver's license required
  • BLS/CPR Certification
  • One to two years of related experience
  • Preferred experience in acute care procedures, especially orthopedics, neurology, cardiopulmonary, and care of burns/open wounds
