Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Homechoice Health Services - Memphis PD. As a Private Duty LPN, you will provide high-quality clinical services to patients in their homes, working under the direct supervision of a Registered Nurse.

Responsibilities
  • Provide direct nursing care to patients in their homes, as prescribed by the physician.
  • Record clinical and progress notes, and coordinate other patient care activities/services to provide the highest quality of patient care.
  • Actively participate in the agency's care coordination process, including timely reporting of patients' needs and changes in condition.
  • Ensure confidentiality of patient health information, and maintain accurate and complete records.
Requirements
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
