Key Responsibilities
  • Deliver high-quality patient care, including assessments, care planning, and interventions, to patients in their homes.
  • Communicate effectively with patients, families, and healthcare providers to ensure comprehensive care and coordination of services.
  • Maintain accurate and detailed records of patient care, including progress notes, medication lists, and treatment plans.
  • Participate in ongoing education and training to stay current on industry developments, new technologies, and best practices in home health care.
Qualifications
  • Current licensure as a Registered Nurse (RN) in the state of Michigan
  • Minimum 1 year of experience as a Home Health Nurse
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
