Key Responsibilities:- Provide targeted, real-time assessments and individual, couples, and/or family consultations and brief interventions.
- Identify, triage, and manage primary care patients with medical and/or behavioral health problems.
- Develop and implement behavioral health care plans, including medication adherence counseling and disease self-management counseling.
- Collaborate with primary care providers to recognize, treat, and manage patients with chronic emotional, psychosocial, substance use, and/or physical health problems.
- Participate in care management processes, including the use of guidelines, disease management techniques, case management, and patient education.
- Triaging and referring patients to specialty mental health care when appropriate.
- Masters in Social Work from a School of Social Work accredited by the Council on Social Work Education or an equivalent degree.
- Licensed Clinical Social Worker (LCSW) or Licensed Clinical Psychologist (PhD; PsyD) in PA.
- A minimum of one year of experience in a behavioral health setting.
