Job Title: Behavioral Health Consultant

Thomas Jefferson University & Jefferson Health is seeking a highly skilled Behavioral Health Consultant to join our team. As a key member of our healthcare team, you will provide behavioral health assessments, brief interventions, and clinical/consultative services to patients and their families in our primary care practices.

Key Responsibilities:
  • Provide targeted, real-time assessments and individual, couples, and/or family consultations and brief interventions.
  • Identify, triage, and manage primary care patients with medical and/or behavioral health problems.
  • Develop and implement behavioral health care plans, including medication adherence counseling and disease self-management counseling.
  • Collaborate with primary care providers to recognize, treat, and manage patients with chronic emotional, psychosocial, substance use, and/or physical health problems.
  • Participate in care management processes, including the use of guidelines, disease management techniques, case management, and patient education.
  • Triaging and referring patients to specialty mental health care when appropriate.
Requirements:
  • Masters in Social Work from a School of Social Work accredited by the Council on Social Work Education or an equivalent degree.
  • Licensed Clinical Social Worker (LCSW) or Licensed Clinical Psychologist (PhD; PsyD) in PA.
  • A minimum of one year of experience in a behavioral health setting.
