Job Title: Home Health Speech Language Pathologist

We are seeking a skilled Speech Language Pathologist to join our team at Alternate Solutions Health Network. As a Home Health Speech Language Pathologist, you will provide high-quality speech therapy services to patients in their homes, helping them to recover from speech and language disorders.

Job Summary:

The successful candidate will have a current license in the state of employment, a minimum of one year of experience as a Speech Therapist in an acute care setting, and valid driver's license and auto insurance. The ideal candidate will be a compassionate communicator with a positive attitude, patience, and attention to detail.

Responsibilities:
  • Develop individualized speech therapy plans for patients and make adjustments as needed.
  • Provide home-based skilled speech therapy care for patients as directed by the attending physician.
  • Teach patients and their families on preventative measures and good decision-making to minimize patient risk.
  • Counsel patients and families on disease/injury and how to manage in all phases of speech therapy.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
Requirements:
  • Speech Therapist with a current license in the state of employment.
  • Minimum one year experience as a ST in an acute care setting. Home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.
