Job Title: Speech and Language Pathologist

ArchCare - Family Home Health Care is seeking a skilled Speech and Language Pathologist to join our team.

Job Summary:

We are looking for a highly motivated and experienced Speech and Language Pathologist to provide speech therapy services to patients in their homes. The ideal candidate will have a strong background in speech and language therapy, excellent communication skills, and the ability to work independently.

Responsibilities:
  • Administer speech and language therapy treatments to patients as per physician's orders using modalities and techniques appropriate for the individual patient.
  • Make an initial and periodic evaluation of the patient's speech and language and rehabilitation needs.
  • Set long and short term goals in measurable terms and adjust treatment plans accordingly.
  • Instruct patients, significant others, and/or ancillary help in the home in activities which can be safely carried out by them between treatment visits.
  • Submit documentation for services provided according to Agency protocol.
  • Make recommendations for referrals to other disciplines when appropriate.
  • Consult with the Rehabilitation Therapies Coordinator and nursing staff, in order to make joint decisions regarding continuation of, or dismissal from care when the need for speech therapist treatment is no longer necessary.
  • Communicate regularly with nurse coordinator per Agency protocol.
  • Assist with the orientation and/or guidance of new staff members, professional and non-professional, nursing students, volunteers, and special visitors.
  • Participate in continuing education programs and provide feedback to assist management in program planning and development.
  • Act as a patient advocate.
  • Participate in selected public relations activities.
  • Participate in selected areas of community health research.
Qualifications:
  • Bachelor's Degree from an accredited Speech Therapy program.
  • One (1) year experience as a Speech and Language Pathologist in a hospital or rehabilitation setting; or equivalent combination of education and experience.
  • Current license to practice as a Registered Speech Language-Pathologist in New York State.
