The Triage Nurse is a vital member of the Hospice interdisciplinary team, responsible for delivering exceptional care through triage calls after hours and on holidays. This role requires strong clinical expertise, effective communication skills, and the ability to prioritize tasks efficiently.
Key Responsibilities- Timely Task Completion: Complete POC tasks promptly, including data entry of referrals, eligibility, scheduling, quick holds, approval of orders, and referrals, all within 30 minutes of request.
- Escalation and Communication: Escalate requests to the manager when a resolution is not identified by the user, and discuss with the manager before escalating to the help desk.
- Task Prioritization: Prioritize incoming tasks according to patient needs, ensuring timely and efficient completion.
- Productivity and Quality: Meet minimum productivity requirements of 100 tasks per day on weekends and holidays, and 50 tasks per day on weekdays, while maintaining high-quality performance.
- Team Collaboration: Participate in department staff meetings and educational sessions to stay updated on best practices and share knowledge.
- Additional Duties: Perform other duties as assigned by the director, contributing to the overall success of the team.
- Education and Experience: Possess an Associate Degree or certification equivalent, with a minimum of 3 years of experience in a related field.
- Licensure and Certification: Hold current RN Licensure for all applicable states assigned prior to accepting calls for agencies.
- Skills and Qualifications: Demonstrate proficiency in HCHB, excellent customer service and communication skills, strong computer skills, and the ability to type.
- Problem-Solving and Analytic Skills: Possess strong problem-solving and analytic skills to effectively resolve complex issues.
- Internet Requirements: Ensure high-speed internet access (not a satellite connection) for efficient task completion.
