MDS Coordinator

We are committed to providing exceptional care to our residents at Seven Oaks Nursing & Rehabilitation. As a key member of our team, the MDS Coordinator will play a vital role in ensuring the highest quality of care and services.

• Schedule residents for assessments and ensure timely completion of the comprehensive assessment (MDS)
• Monitor and maintain accurate documentation of resident care plans and MDS records
• Coordinate and attend regular Medicaid meetings and ITTM meetings to discuss resident care and updates
• Perform care plan updates and provide reports to management and staff
• Ensure compliance with Federal and State regulations regarding MDS and care plan documentation

• Licensed Nurse in good standing
• Geriatric nursing background and restorative care philosophy
• Excellent communication, organization, and time management skills
• Ability to work independently and as part of a team
