Primary Care Clinician

At UnitedHealth Group, we're creating a seamless health journey for patients across care settings. As a Nurse Practitioner or Physician Assistant, you'll be part of our integrated medical team that brings high-end medical service and compassionate care to vulnerable patient populations. You'll play a crucial role in providing annual clinical assessments to patients in their homes, identifying and reducing health risks, and coordinating follow-up care to improve their well-being.
Responsibilities
As a key member of our House Calls team, your primary responsibilities will include:
- Conducting comprehensive in-home assessments, including past medical history reviews, medication reconciliation, vital signs, and physical exams.
- Performing evidence-based practice screenings and point-of-care testing (as necessary).
- Formulating diagnoses for care management and active medical management.
- Communicating findings to inform members' Primary Care Physicians of potential gaps in care.
- Identifying urgent and emergent situations and intervening appropriately.
- Educating patients on topics like disease processes, medications, and compliance.
Requirements
To be successful in this role, you must have:
- An active NP license or ability to obtain one by start date.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification (or the ability to obtain it).
- The ability to complete the physical demands of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
- Reliable transportation for home visit assessments.
Preferred Qualifications
We're looking for candidates with experience in gerontology, cardiology, internal medicine, or endocrinology, as well as those with home healthcare or home visit experience. Ability to travel up to 5% within licensed states for additional incentives is also preferred.
