Required Qualifications:

  • Bachelor's Degree from an accredited Physical Therapy Program
  • Master's Degree from an accredited Physical Therapy Program
  • D.P.T. from an accredited Physical Therapy Program
  • Coursework/Training and occasional specialized physical therapy training

Upon Hire:

  • Oregon Physical Therapist License
  • National Provider BLS - American Heart Association (within 30 days)
