  • Communicate key information regarding inpatient stays and discharge plans to primary care physicians and office-based care managers

Key Responsibilities:

Requirements:

  • Bachelor's degree in nursing from an accredited NLN/CCNE institution
  • 2 years of experience as a Case/Care Manager, Transitional Care Manager, or Nurse Advocate
  • Registered Nurse (RN) licensure in the state and/or compact state
  • BLS certification
