The primary responsibility of the After-Hours Registered Nurse is to ensure the comprehensive and timely provision of hospice care for patients and their families outside of regular business hours, including evenings and weekends.
- Effectively triage patient needs via phone until an in-person visit can be arranged.
- Respond promptly and with urgency to crisis situations from patients and families after hours.
- Conduct initial nursing evaluations to determine eligibility for hospice services during patient visits, within forty-eight (48) hours of referral if assigned.
- Capably perform after-hours visits, including admissions, death visits, recertifications, and routine check-ins.
- Assess the physical, psychosocial, emotional, and environmental needs of patients and families, with reassessments occurring no less than every fifteen (15) days.
- Document identified issues, set appropriate goals, outline interventions, and record patient/family responses to hospice care.
Experience Requirements
- A minimum of one year of clinical experience.
License Requirements
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR Certification.
- Possession of a valid driver's license, vehicle insurance, and access to reliable transportation, or public transit.
Additional State-Specific Requirements
- For Louisiana: A minimum of two years of full-time experience as an RN is required. However, two years of full-time clinical experience in hospice care as a licensed practical nurse may substitute for the required two years of experience as a registered nurse.
