Shift: Days


Key Responsibilities:

  • Collaborate with healthcare teams to develop and implement effective care plans for patients.
  • Conduct thorough assessments of patients' medical histories and current health status.
  • Manage complex discharge planning and facilitate smooth transitions of care.
  • Monitor and evaluate patient progress, adjusting care plans as necessary.
  • Ensure compliance with all hospital policies and regulatory requirements.
  • Provide education and support to patients and their families regarding care plans and resources.
  • Maintain accurate and timely documentation in the MEDITECH charting system.

Qualifications:
  • Must hold a valid Georgia nursing license.
  • Minimum of 5 years of nursing experience, with at least 2 years in a hospital setting.
  • Experience in Utilization Review and Care Transitions is required.
  • Proficient in discharge planning and managing complex cases.
  • Ability to work effectively in various inpatient units (med/surg, ICU, L&D, NICU, Mother/Baby, etc.).
  • Current BLS certification is required.
  • Strong communication and interpersonal skills.

Additional Requirements:
  • Candidates must have a permanent address on their driver's license that is more than 75 miles from Phoebe Putney Memorial Hospital.
  • Applicants must be willing to wait for the necessary licensing process if not currently licensed in Georgia.
