Vista Care is seeking a compassionate and dedicated individual to join our team as a Host Home Provider. As a Host Home Provider, you will have the opportunity to make a meaningful difference in the life of an individual with a disability by providing a safe and supportive living environment.

Key Responsibilities
  • Provide daily living support, including assistance with cleaning, household duties, budgeting, cooking, and medication administration.
  • Teach and assist individuals with daily living skills, promoting independence and self-sufficiency.
  • Manage and schedule medical appointments, ensuring timely and effective care.
  • Provide transportation to medical appointments, community events, and other activities.
  • Document daily activities and progress through a web-based Electronic Health Record.
  • Facilitate community integration and socialization, promoting a sense of belonging and connection.
  • Comply with local, state, and federal regulations, as well as Vista Care's policies and procedures.
  • Communicate effectively with Vista Care representatives and other stakeholders.
Preferred Qualifications
  • Residency in the state where services will be provided.
  • Age 18 or older.
  • Pass a background check, OIG check, sex offender registry check, or other required checks.
  • Adequate homeowner's or renter's insurance.
  • A valid driver's license, reliable transportation, and current vehicle insurance.
  • A reliable computer, internet access, and ability to scan documents.
  • Completion of state-mandated and/or Vista Care required training courses.
