About the Role:
Elite Home Health & Hospice is seeking a skilled Occupational Therapist to join our team. In this role, you will be responsible for implementing standards of care for occupational therapy services and adhering to all conditions outlined in the Service Agreement.

Key Responsibilities:
• Conduct ongoing interdisciplinary assessments and develop individualized plans of care in partnership with patients, representatives, and caregivers.
• Provide services ordered by physicians as indicated in the plan of care.
• Offer patient, caregiver, and family counseling.
• Provide patient and caregiver education.
• Communicate with physicians and other healthcare practitioners regarding the home health plan of care.
• Evaluate patients' functional status and home environments for hazards or barriers to independent living.
• Identify equipment needs and participate in the development of total plans of care.
• Develop treatment programs and establish goals for improved function.
• Teach new skills or retrain patients in daily activities lost due to illness or injury.
• Fabricate splints and instruct patients on adaptive equipment use.
• Train patients in prosthetic and/or orthotic devices.

Requirements:
• Must be licensed where the HHA is located if licensure is required.
• Must have graduated from an OT program accredited by ACOTE and AOTA.
• Must be eligible for or have taken the OT certification examination administered by NBCOT.
• Two years of clinical experience as an occupational therapist.
• Demonstrates good verbal and written communication, and organization skills.
• Possesses and maintains current CPR Certification.
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

Estimated Salary: $85,000 - $110,000 per year
