About the Role

We are seeking a dedicated Youth Community Based Case Manager to join our team at Zepf Center. As a key member of our staff, you will work closely with clients experiencing serious mental illness and/or addiction challenges to provide comprehensive support and coordination of services.

Your primary responsibility will be to assess client strengths and needs, conduct comprehensive care coordination, and develop treatment plans in collaboration with the client and applicable team members. You will also monitor and evaluate client progress toward goal attainment, reviewing and modifying treatment plans as needed.

As a skilled advocate, you will maintain contact with clients, communicate with and provide support to achieve goals and outcomes, and provide active outreach when necessary, including crisis intervention and management. Your exceptional interpersonal skills will enable you to link and refer clients to other services, programs, and agencies as required, ensuring they obtain and retain applicable entitlements.

Key Responsibilities
  • Assess client strengths and needs, conducting comprehensive care coordination and developing treatment plans
  • Monitor and evaluate client progress toward goal attainment, reviewing and modifying treatment plans as needed
  • Maintain contact with clients, communicate with and provide support to achieve goals and outcomes
  • Provide active outreach, including crisis intervention and management, and acquisition of appropriate community supports
  • Link and refer clients to other services, programs, and agencies as required

The ideal candidate will possess excellent communication and problem-solving skills, with a strong ability to work collaboratively as part of a multidisciplinary team. A valid driver's license is essential, as you will be required to safely drive in all-weather conditions in support of client needs.
