About the Role:

We are seeking a highly motivated and compassionate Support Case Worker to join our team at Forestdale Inc. in New York City.

As a key member of our Preventive Service Program, you will work closely with families facing challenges that impact their ability to parent their children. Your primary responsibility will be to assist Case Planners in locating and enrolling families in concrete services and connecting them to resources that address their individual and family needs.

Additionally, you will provide support and guidance to families as they navigate various systems, ensuring they receive the necessary assistance to overcome obstacles and achieve stability.

With your excellent communication and interpersonal skills, you will establish trusting relationships with families, fostering an environment of openness and cooperation.

You will also maintain accurate records, including descriptive narratives of home and office visits, and contribute to program system entries (CARA, Connections, PROMIS). By working collaboratively with Case Planners, you will help clients access essential services, including mental health and substance abuse support.

Salary: $25.38 - $28.95 per hour

Key Responsibilities:

  • Establish regular contacts with families in their homes and build trusting relationships
  • Provide assistance with applications for concrete services, such as fair hearings and public assistance
  • Connect families with community resources and support services
  • Accompany families to appointments and offer guidance in navigating complex systems
  • Help parents improve their skills to create a safe and supportive home environment for their children
  • Maintain accurate records of home and office visits, including descriptive narratives
  • Contribute to program system entries (CARA, Connections, PROMIS)
  • Collaborate with Case Planners to enroll clients in services and monitor their progress

Requirements:

  • Bachelor's Degree in Social Work or related human service field
  • Self-motivated, organized, and compassionate with excellent communication and interpersonal skills
  • Working knowledge of child welfare and family court systems
  • Bilingual (Spanish) strongly preferred
  • Understanding and respect for diverse community needs and cultures
  • Ability to develop strong relationships with children and adults
  • Organizational, verbal, and written communication skills
  • Flexible work schedule, including evenings and weekends, and ability to travel within the metropolitan area
