The Housing Authority of Kansas City, Missouri is looking for dedicated individuals to fill the role of Family Support Case Manager. This position is essential in delivering comprehensive case management services to families who have transitioned from Chouteau Courts. The Family Support Case Manager will evaluate the unique needs of each family, create tailored plans for every family member, and provide necessary resources and referrals. KEY RESPONSIBILITIES
1. Formulates Individualized Care Plans (ICP) that encompass health, education, and employment requirements for each family member.
2. Engages with program participants on a daily basis, utilizing professional judgment to implement the collaboratively developed ICP, aiding residents in reaching their objectives.
3. Supports program participants in locating community resources post-relocation, including job opportunities and high-quality educational institutions.
4. Collaborates with team members and external partner organizations to identify and enhance resources available to families in the Choice Neighborhood initiative.
5. Facilitates access to essential services for program participants through referrals and related activities.
6. Provides crisis intervention strategies to assist program participants in managing crises and recognizing potential triggers for more favorable outcomes.
7. Works in partnership with the HCV department and property management to ensure that program participants possess the necessary documentation for housing applications and recertifications.
QUALIFICATIONS
A Bachelor's or Master's degree in social work, psychology, or a related human services field is required, along with a minimum of five (5) years of experience in case management. Proficiency in computer skills and reliable transportation are also necessary.
