Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at a Residential Facility in Bronx, NY. As a Licensed Practical Nurse, you will be responsible for providing high-quality patient care and working collaboratively with our healthcare team to ensure the best possible outcomes for our patients.

Responsibilities:
  • Provide direct patient care, including administering medications, performing vital signs, and documenting patient information.
  • Collaborate with the healthcare team to develop and implement individualized care plans.
  • Conduct patient assessments and identify potential health risks.
  • Develop and implement education plans to promote patient understanding and self-care.
Requirements:
  • Current NYS LPN License
  • Experience working inpatient or outpatient detox/rehab, residential treatment
  • Current Infection Control Certificate
  • Current BCLS completion card
