Job Summary

Key Responsibilities
  • Collaborate with Physicians and Healthcare Teams: Work closely with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
  • Coordinate Care Transitions: Ensure seamless transitions of care between healthcare settings, including hospitals, clinics, and home care, to prevent readmissions and improve patient outcomes.
  • Manage Utilization and Resources: Analyze and manage the utilization of healthcare resources, including hospital stays, tests, and treatments, to ensure efficient and effective use of resources.
  • Develop and Implement Discharge Plans: Create and implement discharge plans that meet the needs of patients and their families, ensuring a smooth transition to post-acute care settings.
  • Communicate with Patients and Families: Provide education and support to patients and their families, empowering them to take an active role in their care and make informed decisions.
  • Monitor and Evaluate Patient Outcomes: Continuously monitor and evaluate patient outcomes, identifying areas for improvement and implementing changes to optimize care.
Requirements
  • Education and Experience: Bachelor's degree in nursing or related field, with a minimum of 2 years of experience in acute care or case management.
  • Licensure and Certification: Current RN licensure in California, with Basic Life Support certification.
  • Skills and Abilities: Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. Proficiency in electronic medical records and other healthcare software.
