The Adult Case Manager (ACM) plays a crucial role in delivering tailored services to adults facing behavioral health challenges and those who may struggle with independent living. The ACM is tasked with creating a personalized service plan that prioritizes the client's needs, ensuring that all services receive approval through the Administrative Service Organization (ASO) Managed Care System. Additionally, the ACM must keep up-to-date benefit and financial information for clients with cognitive impairments to guarantee the continuity of their benefits.

This role requires adherence to a standard office schedule, working five days a week from Monday to Friday, during the hours of 6am to 5pm. The position also necessitates travel within the assigned area.

Required Qualifications:

Essential Criteria:

  • Valid vehicle insurance with coverage of $50K/$100K and a current Maine driver's license are mandatory
  • Candidates must be at least 18 years old
  • A high school diploma or equivalent is required
  • Certification as MHRT/C or MHRT/CP is necessary
  • Proficiency in computer skills at an intermediate level
