Job Summary

We are seeking an experienced Optometrist to lease space in our Walmart stores and provide high-quality eye care services to our customers. As an independent lease holder, you will have the autonomy to establish your own practice and make decisions about your business.

Key Responsibilities
  • Provide comprehensive eye exams and vision care services to patients
  • Establish your own schedule and hours of operation
  • Set your own fee schedule and keep 100% of exam/professional fees
  • Have the option to opt into equipment/instrumentation and administrative support services (if state allows)
Requirements
  • Doctor of Optometry (OD) from an accredited optometry school
  • Valid state optometry license
  • Ophthalmologist (MD or DO) from an accredited school
  • Valid state ophthalmology license
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • W-9
