Daily Responsibilities:
  • Communicate with the designated care team to maintain plan of care and share changes in patient conditions
  • Assess and form treatment plans for patients with Nephrology-related conditions
  • Work with patients remotely and in the field to provide high-quality care
Required Qualifications:
  • Licensed Nurse Practitioner or Physician Assistant
  • 1-2 years of experience as an NP/PA
  • Ability to meet background check and drug screening requirements
Preferred Qualifications:
  • Experience in Geriatrics, Primary Care, or Nephrology
