Job Summary

We are seeking a dedicated and compassionate Caregiver Coordinator to join our team at L.I.F.E., INC.

About the Role

The successful candidate will be responsible for providing direct support and supervision to individuals with varying abilities, helping them become more integrated in their communities.

This includes assisting with daily needs such as meals, personal care, transportation, leisure activities, and community integration.

  • Preparing and maintaining a detailed weekly schedule of activities for each day of the week to achieve greater independence using person-centered planning and self-determination.
  • Assisting participants with job readiness skills and providing transportation and on-the-job coaching as needed.
  • Planning and supervising work of individuals, individually or in small groups, in the community or on site.

Key Responsibilities:

  • Complete and maintain all state and agency required documentation accurately in accordance with stated guidelines and deadlines.
  • Develop a vocational profile for each individual showing current strengths and weaknesses as related to general job skills and work habits.
  • Maintain positive, professional working relationships through effective communication with individuals, staff, families, and community coordinators for the purpose of collaboration and planning about progress towards personal goals.
Requirements

To be successful in this role, you will need:

  • A High School Diploma or equivalent.
  • Experience in the field of Human Services is a plus but not necessary.
  • Excellent verbal and writing communication skills.
  • Ability to create and maintain efficient working relationships with staff and community partners.
