Job Summary

The Nurse Navigator plays a vital role in ensuring the seamless coordination of patient care. Collaborating with healthcare providers and clinical teams, this professional ensures that patients receive high-quality care and support throughout their treatment journey.

Key Responsibilities
  • Coordinate patient care services to ensure timely and effective treatment
  • Communicate with healthcare providers, schedulers, and other departments to facilitate smooth transitions
  • Assist in tracking incidental radiological and laboratory findings, and ensure patients receive necessary follow-up care
  • Collaborate with the clinic team and other departments to ensure overall coordination of services and positive interactions with providers
  • Assist with survivorship care planning and educate patients on their plan of care
  • Document patient encounters in the electronic medical record and respond to patient phone messages in a timely manner
Requirements
  • Bachelor's degree in Nursing from an accredited nursing school
  • Montana licensure (Registered Nurse)
  • American Heart Association BLS certification
  • 1 year of direct patient care experience
Preferred Qualifications
  • Certification in a specialty area
  • Experience in a specialty area
  • 3 years of direct patient care experience
