About the Role

We are looking for a compassionate and detail-oriented LPN/LVN to provide high-quality patient care in the comfort of our clients' homes. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.

Responsibilities
  • Provide in-home healthcare services to clients with a variety of needs and conditions
  • Develop and implement individualized care plans to meet the unique needs of each client
  • Collaborate with other healthcare professionals to ensure seamless care coordination
  • Communicate effectively with clients, families, and healthcare team members
  • Maintain accurate and detailed records of client care and services provided
Requirements
  • Graduate of a state-approved school of professional nursing
  • Current, unrestricted license as an LPN/LVN in the state(s) of practice
  • Current CPR certification and TB test
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
