About the Role

We are seeking a skilled and compassionate Rehabilitation Nurse to join our team at Wooster Community Hospital. As a Rehabilitation Nurse, you will play a vital role in providing high-quality patient care and supporting our patients on their journey to recovery.

The successful candidate will have a strong background in rehabilitation nursing, with experience in caring for patients with diverse needs and conditions. You will be responsible for assessing patient needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure seamless care transitions.

Key Responsibilities
  • Assess patient needs and develop individualized care plans to support rehabilitation goals
  • Provide direct patient care, including administering medications, performing wound care, and conducting physical assessments
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care planning and coordination
  • Document patient progress and adjust care plans as needed to ensure optimal outcomes
  • Participate in interdisciplinary team meetings to discuss patient care and identify opportunities for improvement
Requirements
  • Degree in Nursing (LPN or RN)
  • Licensure in the State of Ohio
  • Current BLS certification
  • Experience in rehabilitation nursing or long-term care
  • Basic knowledge of rehab nursing and stroke care
Working Conditions

This is a part-time position working 24 hours per week, from 7pm to 7:30am. The successful candidate will be required to work rotating shifts, including nights and weekends.

Compensation and Benefits

We offer a competitive salary range of $48,000-$62,000 per year, depending on experience. In addition to a comprehensive benefits package, including medical, dental, and vision coverage, we also offer paid time off, retirement savings plan, and continuing education opportunities.
