About the Role

We are seeking a skilled Medical Social Worker to join our team at HarmonyCares. As a Medical Social Worker, you will play a vital role in providing high-quality, coordinated healthcare to patients in their homes.

Key Responsibilities
  • Assess and address the social and emotional needs of patients and their families
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with healthcare teams to ensure comprehensive care and support
  • Provide education and resources to patients and families to promote healthy living and disease management
  • Document patient progress and outcomes to inform care decisions
Requirements
  • Master's degree in Social Work from a Council on Social Work Education-accredited program
  • Current Social Worker License in the state of practice
  • Valid driver's license and good driving record
  • Proficiency in clinical skills and ability to make sound professional judgment
  • Excellent verbal and written communication skills
  • Proficiency in personal computer use, including email, clinical, word processing, and spreadsheet software
Preferred Qualifications
  • Advanced Palliative Hospice Social Worker Certification (APHSW-C)
  • Two years of experience in hospice care
  • Certified in Volunteer Administration (CVA)
  • Experience with Electronic Medical Records (EMR)
