Responsibilities:

  • Provide individualized care to residents according to their care plans.
  • Evaluate resident's condition, gather data, and analyze information to assist in the development of the resident care plan.
  • Maintain residents' electronic health records, documenting observations as necessary.

Requirements:

  • Current State license to practice as a Licensed Practical Nurse.
  • A minimum of one year experience working in an acute care, rehabilitation, or post-acute skilled care setting.
  • Ability to sit, stand, lift, bend, reach, push, and pull items weighing up to 50 lbs.
