Fresenius Medical Care Holdings, Inc. is seeking a highly skilled Master Social Worker to join our team. As a Master Social Worker, you will play a vital role in providing psychosocial services to patients treated by our facility, including in-center and home dialysis patients.
Job Summary:The Master Social Worker will work with the healthcare team to promote positive adjustment, rehabilitation, and improved quality of life for our patients. You will assess patients' psychosocial status, strengths, and areas of need that may affect rehabilitation and optimal treatment outcomes. You will also participate in care planning, provide counseling services, and educate patients and their families about kidney disease and its treatment.
Key Responsibilities:- Assess patients' psychosocial status and develop care plans to address their needs
- Provide counseling services to patients and their families
- Collaborate with the healthcare team to promote positive adjustment and rehabilitation
- Provide education and support to patients and their families about kidney disease and its treatment
- Participate in quality improvement activities to enhance patient care
- Masters in Social Work
- State required license
- 0-2 years' related experience
