We are seeking an experienced Advanced Practice Nurse to join our Palliative Care Team at ProHealth Care. This is a dynamic, collaborative, and supportive team that provides patient-centered care at all three ProHealth campuses. The ideal candidate will have a Master's degree in Nursing, be licensed as a Nurse Practitioner, and certified by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP). In this role, you will work closely with patients, families, and healthcare teams to provide comprehensive care and support.

About the Role

In this role, you will:

  • Assess, diagnose, and treat patients under your care, managing acute and chronic illnesses, including co-morbidities.
  • Obtain medical histories, ensure relevant documentation, and interpret patient care data to determine management and treatment plans.
  • Order and interpret diagnostic studies, perform minor surgeries and routine procedures, and prescribe pharmacologic and non-pharmacologic therapies as needed.
  • Provide coordination of referral services, manage ancillary services, community resources, and preoperative plans.

You will also be responsible for:

  • Encouraging patient involvement in care through counseling, instructing, and educating.
  • Promoting active discussion with patients regarding plan of care, treatment options, care outcomes, preventive health measures, and self-care skills.

The estimated salary for this position is $120,000-$150,000 per year, depending on experience.
