About the Role

We are seeking a skilled and compassionate LPN to join our team as a Private Duty Nurse. As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes, working closely with our patients and their families to ensure the highest level of care and support.

Responsibilities
  • Implement and manage physicians' prescribed plans of care
  • Conduct patient assessments and coordinate care with other healthcare professionals
  • Administer prescribed medications, treatments, and therapies
  • Document care provided in our electronic system of record
  • Provide health education and training to patients and their families
Requirements
  • Active, unencumbered LPN license in the state of practice
  • Current CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within the last 12 months)
  • At least six months of prior hands-on nursing experience (preferred but not required)
  • Reliable transportation
