Nurse Care Specialist

We are seeking a compassionate and skilled Registered Nurse (RN) to join our team in providing high-quality home health care to our clients in Kent County, DE.
About the Role:
- Provide comprehensive home health care services to adult and geriatric clients in their homes.
- Assess clients' needs and develop personalized care plans to ensure their comfort, independence, and dignity.
- Collaborate with clients' healthcare teams to ensure seamless care transitions.
- Document client interactions, assessments, and care plans accurately and thoroughly.
Requirements:
- A current license as a Registered Nurse in Delaware.
- Prior home care experience is strongly preferred, but not required.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills and ability to communicate effectively with clients and their families.
