Job Overview

Position Title: Care Partner

Objective:
To deliver personal assistance and support in monitoring the health and living conditions of residents receiving assisted living services.

Qualifications:

• Certified Nursing Assistant (CNA) or relevant experience in elderly care within a residential or long-term care environment is preferred.

• Valid CNA license, or other applicable certification, or the ability to acquire as mandated by state regulations.

• Capability to measure vital signs such as blood pressure, pulse, respiration, temperature, and oxygen saturation, or willingness to learn.

• Must maintain current CPR and First Aid certifications.

• Must hold valid Food Handlers certification.

• Must complete annual Continuing Education requirements.

• A passion for working with senior citizens is essential.

• Ability to foster positive relationships with residents, their families, and team members.

• Proficient in reading, writing, and speaking English to communicate effectively with residents, families, visitors, and colleagues.

• Basic computer skills, including familiarity with Word and Excel, are required.

• Must represent the community positively and professionally.

• Compliance with all health-related requirements as per state licensure.

• Ability to perform essential job functions with or without reasonable accommodation.

• Subject to pre-employment screenings including physical examination, drug testing, and background checks.

Key Responsibilities:

Resident Support

• Familiarize with and comprehend all individual service plans for residents.

• Assist residents with daily living activities such as bathing, dressing, grooming, toileting, eating, transferring, and mobility as outlined in their service plans.

• Engage residents in meaningful conversations and social interactions while providing personal care.

• Monitor residents for any changes in their physical, emotional, mental, or social conditions, unusual symptoms, accidents, or incidents, and report promptly to a licensed nurse or supervisor.

• Complete necessary documentation, including incident reports and other written records, to ensure effective communication among the Resident Care Team and licensed nurses.

• Support residents during mealtimes in the dining area, serving meals with proper techniques and ensuring an appealing presentation.

• Encourage resident participation in scheduled activities and outings, providing assistance as needed.

• Where permitted by state regulations, assist with postmortem care conducted by a Medication Technician for residents who pass away in the community under hospice care.

Housekeeping Duties

• Make resident beds, dispose of trash, and tidy resident apartments daily or as requested.

• Prepare resident beds for the evening if requested.

• Change linens and towels according to the community's laundry schedule and as needed.

• Keep resident clothing and closets organized and neat.

• Assist with scheduled and unscheduled resident laundry as necessary.

• Notify housekeeping staff of spills and soiled areas, assisting with clean-up as required. If no housekeeper is available, address spills and soiling immediately.

• Help clean mobility aids and other resident equipment as needed.

• Perform additional housekeeping tasks as assigned.

Additional Responsibilities

• Attend all mandatory team meetings and training sessions.

• Answer phones in a courteous and timely manner as needed.

• Uphold residents' rights as defined by state regulations and company policies at all times.

• Support and adhere to all company policies and service philosophies.

• Follow the company dress code while on duty.

• Maintain reliable and predictable attendance.

• Perform other duties as assigned.
