Job Summary

We are seeking a highly skilled and compassionate Licensed Therapist to join our Families Are Made (FAM) program. As an In-Home Mental Health Therapist, you will work with clients in their homes, providing therapy services to individuals and families in need. This is a part-time fee-for-service position, with a focus on delivering high-quality care to our clients.

Key Responsibilities
  • Conduct standardized counseling/talk therapy sessions for Stokes Counseling clients
  • Perform intake/biopsychosocial assessments to evaluate clients' mental and behavioral health status
  • Formulate differential diagnoses along with short-term and long-term treatment plans tailored to individual client's needs
  • Provide timely and professional counseling services to scheduled clients utilizing professional counseling industry standards
  • Identify and refer clients with urgent mental health conditions to appropriate emergency or psychiatric treatment facilities
  • Provide and maintain written documentation of intake, assessments, progress/contact notes, interventions, treatment plans, and termination notes within the designated time frame
  • Maintain an understanding of how behavioral health priorities are administered under the State of Connecticut and consult with specialists and colleagues to review and participate in the care of clients
  • Maintain appropriate documentation of clinical and counseling services, including all necessary billing information
Requirements
  • Fully licensed mental health therapist in the State of Connecticut (LPC, LMFT, or LCSW)
  • One to three years of experience conducting counseling/talk therapy sessions with diverse client populations and children
  • Advanced proficiency in conducting intake assessments and formulating treatment plans
  • Strong written communication and interpersonal skills, including the ability to provide and maintain documentation within the appropriate time frames
  • Ability to work collaboratively in a team-oriented environment
  • Commitment to maintaining the highest ethical and professional standards in clinical practice
  • Experience with specializations such as TFCBT, EMDR for Trauma, DBT, Eating disorders, and working LBGTQIA+ community is strongly preferred
  • Passion for in-home work
  • Have a valid driver's license and access to a properly insured vehicle for work or reliable transportation
