Naamans Creek Country Manor - Powerback is seeking an experienced Physical Therapist to join our team in a Skilled Nursing Facility setting. As a Physical Therapist, you will be responsible for managing a treatment plan to promote a positive outcome for patients with diverse diagnoses.

Job Summary:

We are looking for a Physical Therapist with a strong background in Skilled Nursing Facility settings to join our team. The ideal candidate will have a minimum of one year of recent experience in an SNF setting and be able to manage a treatment plan to promote a positive outcome for patients with diverse diagnoses.

Responsibilities:
  • Manage a treatment plan to promote a positive outcome for patients with diverse diagnoses
  • Work in a Skilled Nursing Facility setting
  • Collaborate with other healthcare professionals to provide comprehensive care
  • Document patient progress and treatment plans
Requirements:
  • Current BLS (AHA Preferred)
  • Active State License
  • Supervisory Professional References
  • Must complete Drug Screen and Background Screen
