The successful candidate will be responsible for providing rehabilitative treatment for patients with cognitive and/or ADL physical functioning disorders, adhering to regulatory requirements. They will also screen and evaluate patients, developing care plans following regulatory and clinical standards. In addition, they will utilize standardized assessments and evidence-based practices to support clinical interventions.

We offer a competitive salary of $50,400 - $54,800 per year, based on full-time employment status. Responsibilities:
  • Develop and implement individualized care plans for patients with cognitive and/or ADL physical functioning disorders
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support
  • Conduct standardized assessments and utilize evidence-based practices to support clinical interventions
  • Monitor patient progress and adjust treatment plans as needed
  • Communicate effectively with patients, families, caregivers, and other service providers
Qualifications:
  • OCCUPATIONAL THERAPY LICENSE REQUIRED
  • Experience working in long-term care or similar settings
