Responsibilities:
Develop, review, and revise plans of care in collaboration with clients, families, and healthcare teams.
Provide skilled nursing care in accordance with physician orders and individual plans of care.
Observe, record, and report client reactions to treatment and changes in condition to appropriate personnel and/or physicians.
Educate clients, families, and healthcare teams on plan of treatment and provide ongoing supervision and support.
Monitor and record patient condition and document provided care services via electronic charting.
Participate in education programs, quality assessment activities, and staff meetings as required.
Requirements:
Active Licensed Practical Nurse or Registered Nurse License.
Strong problem-solving skills and ability to multitask.
Excellent communication skills.
