About the Job
In this position, you will provide guidance and support to our clinicians in the post-acute care setting. You will also be responsible for conducting regular clinical case reviews, developing strategies to improve patient outcomes, and collaborating with other leaders to advance our care coordination initiatives.
Key Responsibilities
- Provide guidance and support to our clinicians in the post-acute care setting.
- Conduct regular clinical case reviews to identify areas for improvement.
- Develop and implement strategies to improve patient outcomes.
- Collaborate with other leaders to advance our care coordination initiatives.
- Work closely with pharmacists to optimize medication management.

Requirements
- Strong leadership and communication skills.
