Palliative Care Nurse Practitioner Opportunity

Trinity Health Of New England Medical Group is seeking a skilled and compassionate Nurse Practitioner to provide specialist-level palliative care as part of our Palliative Care team at Saint Francis Hospital and Medical Center in Hartford, Connecticut.

About the Role
  • Collaborate with clinical staff to provide comprehensive palliative care services to patients and their families facing life-threatening illnesses.
  • Provide education and psychosocial support, address goals of care, and manage pain and non-pain symptoms.
  • Assist in coordination of care for patients transitioning to home hospice/home palliative care.
  • Utilize our analytics to audit and improve patient care.
  • Develop and implement care plans that meet the unique needs of each patient.
  • Work closely with the Palliative Care team to ensure seamless care coordination.
