Job Description
We are seeking an experienced Street Homeless Outreach Coordinator to join our team. This role involves providing outreach services to street homeless individuals, connecting them with shelter, services, and housing. The successful candidate will work closely with community resources and stakeholders to ensure clients receive comprehensive support.
About the Role
The Street Homeless Outreach Coordinator will be responsible for:
- Providing outreach services to street homeless individuals, including interviews and orientations
- Collecting information necessary for intake processes, including employability and stable housing recommendations
- Recording case information, updating files, and providing status updates to supervisors
- Conferring with supervisors and team members at staff meetings to ensure proper case management
- Collecting and maintaining data related to reporting requirements
- Providing community liaison services as assigned by supervisor
- Transporting clients to shelters or other destinations
- Conducting assessments to determine client vulnerability and self-sufficiency
- Completing data collection and entering it into Client Track and OATS systems within 2 business days
- Assisting clients with accessing public benefits and assistance
- Connecting or referring clients to services provided at shelter service centers
- Making timely and accurate decisions in emergency or crisis situations
- Providing street-based case management to unhoused populations
- Providing access to emergency shelter, meals, showers, and laundry services
Required Skills and Qualifications
To succeed in this role, you will require:
- A Bachelor's degree and 3-5 years of experience in mental health or addictions treatment
- Personal or professional experiences with homelessness
- Basic knowledge of addictions treatment and/or mental health treatment programs
- General understanding of substance abuse and/or mental health programs
- Knowledge of highly utilized locations in Polk, Hardee, or Highlands counties where homeless congregate and camp
- Ability to walk long distances and on rough terrain
- Ability to lift and carry outreach supplies (up to 30 lbs.)
- Understanding of and sensitivity to the needs of the homeless
- Commitment to increasing housing stability and self-sufficiency
- Strong verbal and written skills
- Good problem-solving skills and ability to prioritize multiple tasks
