Behavioral Health Case Manager

As a Behavioral Health Case Manager at Zufall Health, you will play a vital role in assisting patients and their families in accessing appropriate clinical and social services. This position requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Key Responsibilities
- Work as part of the Care Team to identify patients in need of services and communicate with providers as to progress.
- Familiar with hospital systems, such as Epic or eCW, and have knowledge of Psych medications.
- Excellent time management skills and ability to prioritize tasks effectively.
- Keep an active problem list and track patients that have been referred, documenting level of services provided and issues reports on a monthly basis.
- Follow up on missed appointments and reschedule as appropriate, contacting patients that are overdue for visits.
- Ability to receive and provide feedback and communicate effectively with the team and superiors.
- Enter interventions and follow-up plans in electronic medical records.
- Assist patients with arrangements for transportation, translation, and other barriers to attending appointments.
- Assist patients with completion of medical and prescription forms and requests to other providers and agencies.
- Facilitate referrals for Zufall Health Center providers to refer patients into behavioral health and enabling services, including referrals to outside agencies and programs.
- Identify community resources and services that address barriers encountered by patients and their families and assist patients in accessing such services.
- Act as an advocate for patients and their families with community organizations and other agencies.
Requirements
- Minimum Associate Degree, or a minimum of three years of social services-related experience and pursuing a degree in a social services field.
- Minimum two years of experience in a clinical setting (preferred).
- Experience working with community service agencies.
- Knowledge of Microsoft Office, keyboarding, and use of electronic medical records.
- Bilingual English/Spanish required.
