Job Title: Registered Nurse

LHC Group is seeking a skilled Registered Nurse to join our team in Richmond, Kentucky. As a Registered Nurse, you will provide high-quality patient care in the comfort of their own homes.

About the Role:
  • Provide comprehensive nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws and regulations.
  • Coordinate total plan of care with other healthcare professionals involved in patient care.
  • Develop and implement individualized care plans, ensuring continuity of patient care.
Responsibilities:
  • Conduct initial and comprehensive nursing evaluations, determining primary focus of care and developing plans accordingly.
  • Regularly evaluate and re-evaluate patient nursing needs, making necessary revisions to care plans.
  • Ensure patients meet home health eligibility and medical necessity guidelines.
  • Collaborate with physicians and other healthcare professionals to develop and implement care plans.
Requirements:
  • Current RN licensure in the state of Kentucky.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
