Responsibilities

As a RN Care Specialist, you will be responsible for:

  • Performing patient start of care comprehensive assessments, evaluations, and implementing plans of care.
  • Completing OASIS assessments timely and accurately.
  • Providing skilled nursing care to patients in accordance with the plan of care and agency policies.
  • Helping to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members, and community resources.
  • Initiating appropriate referrals when indicated.
  • Promoting and maintaining patient health through teaching and appropriate rehabilitative measures.
Qualifications

To be considered for this role, you must have:

  • A current RN license, specific to the state(s) you are assigned to work.
  • One year of clinical experience as an RN.
  • Current CPR certification.
  • A valid driver's license, reliable transportation, and liability insurance.
