Job Summary

We are seeking a compassionate and skilled Nurse Practitioner to join our team. As a Medical Services Coordinator, you will provide evidence-based medical care to seniors and patients with complex needs.

Key Responsibilities
  • Performs comprehensive assessments and develops individualized care plans.
  • Orders and interprets laboratory and diagnostic testing as needed.
  • Collaborates with caregivers and other healthcare professionals to provide interdisciplinary care.
  • Provides referrals to other healthcare practitioners when necessary.
Requirements
  • Active state Nurse Practitioners license.
  • Current Nurse Practitioner National Board Certification.
  • Master's degree in Nursing (MSN).
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
