About the Role

We are seeking a skilled Nurse Practitioner to join our team in delivering preventive care and educational services directly to our members in their homes. As a Nurse Practitioner at EasyHealth, you will play a vital role in performing comprehensive assessments, such as Annual Wellness & Quality Gap Care visits, and providing personalized care to our members.

Key Responsibilities
  • Perform clinical screenings and various tests in an in-home setting
  • Accurately code, verify and document clinical findings
  • Complete a detailed evaluation of the medical and social assessment of the beneficiary members using our proprietary tool or client EMR
  • Identify and refer members/patients for routine, urgent, and emergent case management
  • Make recommendations for member's follow-up with their primary care physician
Requirements
  • 2 years hands-on experience as a Nurse Practitioner
  • Current, unrestricted license in the state(s) in which the services are rendered
  • Board certified, preferably geriatric or adult medicine specialty
  • Familiarity with HCC model and quality HEDIS and other STAR measures
  • Effectively communicate with elderly and chronically ill members
  • Proficient in use of computer, tablet, and mobile technology
  • Willingness to travel within geographic area
