Job SummaryWe are seeking a compassionate and skilled Physical Therapist to join our team of healthcare professionals. As a Physical Therapist at Pinnacle Home Care Inc., you will have the opportunity to work with patients in their homes, providing personalized care and support to help them achieve their rehabilitation goals.
Responsibilities- Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
- Develop and implement individualized treatment plans in consultation with physicians and other healthcare professionals.
- Provide physical therapy services to patients in their homes, including exercises, stretches, and other interventions to promote mobility and function.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Supervise and train Physical Therapist Assistants as needed.
- Document patient progress and outcomes in accordance with company policies and procedures.
- Graduate of an accredited Physical Therapy program or equivalent education.
- Licensed to practice as a PT in the State of Florida.
- Current BLS Certification.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
- At least one year of experience as a Physical Therapist.
- Home healthcare experience is preferred but not essential.
- Electronic medical record experience is preferred but not essential.
