Key Responsibilities
  • Assessment and Planning: Conduct thorough psychosocial assessments to identify patient needs and develop individualized treatment plans.
  • Care Coordination: Collaborate with outside providers to ensure seamless care coordination and communication.
  • Patient Advocacy: Act as a patient advocate to coordinate required services, resolve emergency problems, and ensure patient confidentiality.
  • Documentation and Communication: Maintain accurate and timely documentation of patient interactions and communicate effectively with multidisciplinary teams.
  • Professional Development: Participate in ongoing care collaboration meetings, staff meetings, and professional development opportunities to enhance skills and knowledge.
Qualifications and Requirements
  • Education: Bachelor's Degree in counseling, social work, or other allied mental health field.
  • Experience: One-year experience working in the substance abuse treatment field and one-year experience working in the mental health counseling field.
  • Licensure: MHRTc license required.
Preferred Skills
  • Professionalism: Maintain professionalism in the workplace, including professional boundaries and codes of ethics.
  • Communication: Excellent verbal and written communication skills, with the ability to present information in a group setting.
  • Time Management: Effectively manage time to meet deadlines and prioritize tasks with limited supervision.
  • Electronic Health Records: One-year experience using an Electronic Health Record, with previous use of Athena Electronic Health Record preferred.
